Forehead Lines: What Wrinkle Relaxer Can (and Can't) Do
Horizontal lines across the forehead are one of the most common concerns patients bring up at Blanqa — and also one of the treatments people are most nervous about, usually because they've seen a photo somewhere of an overdone, flat, "frozen" forehead. The good news is that outcome isn't inevitable; it's almost always a function of technique, not the medication itself. Forehead lines form from the frontalis muscle, the same muscle that raises your eyebrows, and treating it well means relaxing the lines without flattening your natural expression. Here's what the research actually shows about wrinkle relaxer for this area, and how a thoughtful injection plan avoids the look patients worry about.
Why Forehead Lines Form
A few things tend to combine to create these lines over time:
• Frontalis overuse. Raising your eyebrows to express surprise, focus, or emphasis repeatedly folds the skin across the forehead — and many people do this far more often than they realize, especially during long days of screen work or squinting into bright sun.
• Compensatory brow lift. Some people unconsciously use their frontalis to hold their eyelids up if their brows sit lower or their upper eyelids are heavier, which increases forehead muscle activity even further.
• Sun exposure and collagen loss. As with other areas of the face, years of UV exposure common in the Las Vegas Valley break down collagen and elastin, making lines set in more permanently.
• Natural aging of the skin. Skin gradually loses thickness and elasticity, so creases that used to disappear when the face was at rest start to linger.
How Wrinkle Relaxer Targets the Forehead
Wrinkle relaxer works here the same way it does elsewhere: it temporarily reduces the nerve signal to the frontalis muscle so it contracts more gently. The forehead is one of the more technique-sensitive areas to treat, though, because the frontalis also helps support brow position — treat it too aggressively or unevenly and brows can feel heavy or sit asymmetrically. A good injection plan spreads smaller doses across multiple points rather than concentrating a large dose in a few spots, which is part of why an experienced injector matters more here than in some other areas.
What the Research Shows
Forehead lines are one of the better-documented areas in the neuromodulator literature. A few key findings:
• A 2026 GRADE-assessed systematic review and meta-analysis of incobotulinumtoxinA for upper facial lines found a statistically significant treatment benefit for horizontal forehead lines at Day 30 compared with placebo, with response rates dramatically higher in treated patients across every measure the researchers tracked (investigator- and participant-assessed). The pivotal trial in that review used 54–64 units of incobotulinumtoxinA and reported a 70.9% responder rate for forehead lines at Day 30, with no statistically significant increase in headaches or other adverse events compared with placebo (Azam et al., Aesthetic Surgery Journal Open Forum, 2026).
• Multiple trials in this same body of research have specifically studied the forehead alongside the glabella (the "11" lines between the brows), since many patients are treated in both areas together — findings across these studies consistently show that treating both regions in a coordinated way, rather than forehead alone, gives a more balanced, natural-looking result and helps prevent the compensatory brow-heaviness some patients notice when only the forehead is treated.
• The American Society of Plastic Surgeons lists forehead furrows among the standard, FDA-approved indications for botulinum toxin treatment, alongside crow's feet and frown lines, using the same nerve-blocking mechanism across all three areas.
To be transparent about the limits: results depend heavily on injector technique and an individualized dosing map, more so in this area than almost anywhere else on the face. This is also an area where "more is not better" — overtreating the forehead is the most common cause of the heavy, flat look patients want to avoid, which is why a conservative starting dose with room to adjust at a follow-up is usually the safer approach for a first treatment.
Am I a Good Candidate?
• Good candidates typically have: visible horizontal lines with expression, brows that sit in a reasonably balanced position at rest, and realistic expectations about softening rather than eliminating lines.
• Caution or non-candidacy includes: pregnancy or breastfeeding, neuromuscular conditions such as myasthenia gravis, very heavy or low-set brows that may need a different treatment plan, active skin infection at the injection site, or known hypersensitivity to the product.
• A combined forehead-and-glabella plan is often recommended over treating the forehead alone, specifically to keep brow position balanced.
What to Expect: Timeline
• Days 1–3: Mild redness, small bumps, or pinpoint bruising possible at injection sites.
• Days 3–7: Early softening of lines with expression often begins.
• Day 14: Full effect is generally visible; this is the right time to assess whether brow position and expression feel natural.
• 3–4 months: Typical duration before a touch-up is needed to maintain results, consistent with other upper-face neuromodulator research.
