Can Microneedling Actually Fade Stretch Marks? What the Research Shows
“Will these ever go away?” It's one of the questions I hear most often at Blanqa — usually about stretch marks left behind after pregnancy, a growth spurt, a season of weight change, or building muscle. Stretch marks (the clinical term is striae distensae) are one of the most common skin concerns we see, and also one of the most misunderstood. Patients are often sold creams that do very little, so it's fair to be skeptical of anything else that claims to help — including microneedling. Here's what the current research actually shows.
What Causes Stretch Marks in the First Place?
Stretch marks form when the skin is stretched faster than its collagen and elastin fibers can adapt, combined with hormonal shifts that change how those fibers are made and maintained. The result is a tear-like disruption in the deeper layer of skin (the dermis) that eventually heals into a scar-like band.
Common contributing factors include:
• Pregnancy — by far the most common cause, affecting a large majority of pregnant women
• Puberty growth spurts, especially rapid height or weight gain in a short window
• Rapid weight loss or weight gain
• Fast muscle growth from strength training
• Long-term corticosteroid use (topical or oral), which weakens collagen support
• Family history — genetics play a real role in who develops stretch marks and how visible they become
Because the cause is mechanical stretching plus a hormonal/genetic component, stretch marks can affect people at a healthy weight, athletes, and teenagers alike. They are not a sign that something is wrong with your skin or your effort.
Two Very Different Stages: Rubrae vs. Albae
Not all stretch marks are the same, and the stage matters for what kind of results are realistic.
• Striae rubrae (early/new): pink, red, purple, or reddish-brown; sometimes slightly raised or itchy; still actively inflamed with visible blood vessels
• Striae albae (mature/older): faded to white, silvery, or skin-toned; flatter and thinner-feeling; less blood flow to the area, which makes them respond more slowly to treatment
In general, earlier (rubrae) stretch marks tend to respond better to treatment than long-established (albae) ones, simply because there's more active tissue to work with.
Where Microneedling Fits In
Microneedling uses a device with fine, sterile needles to create controlled micro-injuries in the skin. That controlled injury triggers the body's own wound-healing response — new collagen and elastin production, and remodeling of the existing scar-like tissue in a stretch mark. It's one of the reasons Microneedling is one of our most requested services at Blanqa, alongside its use for acne scarring and texture.
Unlike topical creams, which mostly sit on the skin's surface, microneedling works in the same deeper layer where the original damage occurred — part of why the research on it looks more encouraging than the research on lotions and oils.
What the Research Shows
I want to be upfront: stretch marks are considered a form of scarring, and no treatment — including microneedling, lasers, or anything else — makes them disappear completely. What the evidence supports is meaningful improvement in appearance, not erasure. With that framing:
• Sun, Jia & Huang (2024, Aesthetic Plastic Surgery): a systematic review and meta-analysis pooling 11 studies (6 randomized controlled trials and 5 non-randomized studies) found microneedling performed as well as laser treatment, with no statistically significant difference (P = 0.35), and significantly outperformed non-laser comparison treatments (P < 0.0001).
• Same review, safety finding: microneedling caused noticeably less post-inflammatory hyperpigmentation than other treatment types (P = 0.0003) — an important consideration for patients with deeper skin tones, who are more prone to that side effect from laser-based options.
• Same review, satisfaction: patient satisfaction with microneedling was rated significantly higher than with laser (P = 0.04) and non-laser treatments (P = 0.04), though microneedling caused more discomfort during treatment than laser (P < 0.00001).
• Kaur et al. (2024, Journal of Cutaneous and Aesthetic Surgery, n=30): a randomized trial comparing microneedling alone to microneedling plus the patient's own platelet-rich plasma (PRP) over three sessions found the combination group had significantly better Manchester Scar Scale scores (6.87 vs. 9.60, P = 0.01) and higher “very satisfied” rates (86.6% vs. 73.3%, P = 0.02).
• NCBI clinical overview (Striae Distensae): describes the mechanism behind microneedling's benefit as stimulation of fibroblast activity and new collagen synthesis in the same dermal layer where the original stretching occurred.
The authors of the 2024 meta-analysis themselves described the evidence as an encouraging “initial” body of research (rated Level II evidence) — solid, but not yet as large or long-term as we'd like. Many supporting studies are relatively small, and several combine microneedling with an add-on like PRP rather than isolating microneedling alone. Worth saying plainly rather than overselling the data.
Is Microneedling a Good Fit for You?
Good candidates for microneedling for stretch marks generally:
• Have realistic expectations — visible fading and softer texture, not disappearance
• Have stretch marks in the rubrae (pinker, newer) stage, though established albae marks can still see texture improvement
• Are not currently pregnant or breastfeeding (we wait until after, for timing and comfort)
• Have no active skin infection, open wound, or recent isotretinoin use in the treatment area
• Are able to commit to a short series of sessions rather than expecting results from one visit
Microneedling may not be the right first step if you have a history of keloid scarring, an active flare of eczema or psoriasis in the area, or a bleeding disorder that would also complicate a PRP add-on. We screen for all of this at your consultation before recommending a plan.
What to Expect: A Realistic Timeline
• Session cadence: most protocols use a series of 3 to 6 sessions, spaced about 4 weeks apart, to allow full healing and collagen remodeling between visits
• Immediately after: mild redness and pinpoint sensitivity for 24 to 48 hours, similar to a light sunburn
• Early changes: subtle improvements in texture and color can start to appear 4 to 6 weeks after a session, as new collagen forms
• Full course results: most visible improvement is typically assessed 3 to 6 months after the final session in a series, once remodeling has settled
Because Henderson and Las Vegas summers mean a lot of pool days and swimsuits, many patients like to start a microneedling series in fall or winter, spacing out sessions so results develop before the next warm season.
