NAD+ IV Therapy: What the Research Actually Shows

NAD+ IV therapy has become one of the most requested treatments in wellness medicine — and one of the most hyped. Search interest keeps climbing as biohackers and wellness influencers credit it with everything from sharper focus to slower aging. Here at Blanqa Medspa in Henderson, NV, patients ask about it nearly every week, and we offer both a direct NAD+ IV and Niagen® IV, a nicotinamide-riboside-based option.

But NAD+'s popularity has outpaced the research behind it. So instead of writing another post about why you need this treatment, we want to do something more useful: walk through what the clinical evidence actually says right now — including where it's thin — so you can decide alongside your provider, not a marketing claim.

What Is NAD+, and Why Does It Matter?

NAD+ (nicotinamide adenine dinucleotide) is a coenzyme found in every cell in your body, where it plays a central role in converting food into cellular energy and supporting DNA repair. Because NAD+ activity is tied to metabolism, researchers have long been interested in whether replenishing it through IV infusion, injections, or oral precursors could support energy, cognitive function, or healthy aging.

Here's where the science gets more cautious than the marketing suggests. A 2025 review in Nature Metabolism found that evidence for an age-related decline in NAD+ levels in humans "has been consistently observed only in a limited number of studies" — meaning the basic premise many NAD+ therapies are built on isn't as settled as often claimed. A newer 2026 study from the same research group measured whole-blood NAD+ across seven independent human cohorts and found levels "remain remarkably stable across age and lifestyle interventions," challenging the idea of blood NAD+ as a marker of aging at all.

What the Research Shows

•          Nature Metabolism (2025), Vinten et al. — A review of NAD+ precursor supplementation in human aging concluded that although animal studies are promising, human clinical trials "have shown limited efficacy," and called for more rigorous, tissue-specific research before drawing conclusions about anti-aging benefits.

•          Nature Metabolism (2026), Trętowicz et al. — A direct measurement study across seven human cohorts (over 300 participants combined) found whole-blood NAD+ levels did not meaningfully differ by age or lifestyle factors, though levels did respond as expected to nicotinamide riboside supplementation, confirming the measurement method itself was sound.

•          medRxiv pilot RCT (2024) — A randomized, placebo-controlled pilot study compared IV NAD+ against IV nicotinamide riboside (marketed as Niagen®), oral nicotinamide riboside, and saline placebo in healthy adults. IV nicotinamide riboside raised blood NAD+ levels by roughly 21% at three hours — notably more than IV NAD+ itself — while finishing in about a quarter of the infusion time. This is a preprint and has not yet completed peer review.

•          Frontiers in Aging (2026) retrospective study — A real-world review of clinic records comparing IV NAD+ to IV nicotinamide riboside over four consecutive infusion days found that every patient in the small NAD+ group experienced infusion-related side effects (cramping, nausea, or vomiting), compared with roughly six in ten in the nicotinamide riboside group, whose symptoms were milder.

These comparative studies were small and short-term — the two head-to-head trials combined fewer than 100 participants — so they're best read as hypothesis-generating, not definitive. But paired with the aging-biomarker research above, they support real caution about the bigger claims made for NAD+ IV.

NAD+ IV vs. Niagen® (NR) IV: What Actually Differs

Because we offer both formulations, patients often ask which one makes more sense. Based on the research above:

•          Infusion time: direct NAD+ IV has run considerably longer per session in published data (close to 90–100 minutes) than Niagen®/NR IV (closer to 25–40 minutes).

•          Tolerability: NAD+ IV has been associated with more nausea, cramping, and headache during infusion in the small studies above; Niagen®/NR IV was generally better tolerated, with milder tingling reported by some patients.

•          Blood NAD+ response: in the 2024 pilot study, IV nicotinamide riboside produced a larger short-term rise in measured blood NAD+ than IV NAD+ itself.

•          Bottom line: neither option is proven to deliver the sweeping anti-aging or cognitive benefits sometimes advertised, but if you want to try this category of treatment, the current data is a reasonable argument for starting with Niagen® IV and discussing your goals with your provider first.

Who Might Be a Candidate — and Who Should Hold Off

•          Best suited for generally healthy adults who understand this is a wellness/preventive treatment, not a treatment for a diagnosed medical condition.

•          Should be avoided, or discussed carefully with your primary care provider first, if you are pregnant or breastfeeding, since NAD+ IV therapy has not been studied in these populations.

•          Warrants extra caution for anyone with significant kidney or heart conditions, given the fluid volume involved in IV infusions.

•          Not a substitute for evaluating fatigue, low energy, or brain fog with a qualified provider first — these symptoms can have underlying causes such as thyroid disorders, anemia, or sleep problems that deserve proper diagnosis rather than an IV alone.

•          Should be disclosed to your provider alongside your full medication list, since interactions between high-dose IV nutrient therapy and prescription medications haven't been thoroughly studied.

What to Expect During and After Treatment

•          Before your first infusion: a consultation and health history review to confirm you're a reasonable candidate.

•          During infusion: plan for roughly 45 minutes to just over an hour depending on the formulation and dose; mild nausea, flushing, or cramping can occur and typically resolves quickly when the infusion rate is slowed.

•          Right after: most patients feel back to normal quickly and can resume regular activities the same day.

•          In the days that follow: any subjective effects are individual and are not guaranteed by current research — we recommend tracking how you feel across a few sessions rather than judging from a single visit.

We offer NAD+ IV and Niagen® IV because patients ask for them, and we believe adults deserve access to treatments they're curious about — paired with accurate information. What we won't do is promise you'll feel ten years younger or present NAD+ therapy as settled science when the honest answer is that it's still emerging.
