PDO Thread for Skin Laxity: What the Research Actually Shows

If you've researched non-surgical facelift options, you've likely come across PDO thread lifts — sometimes marketed as a “lunchtime facelift.” The pitch: dissolvable threads placed under the skin that lift sagging tissue and stimulate collagen, without surgical downtime. Here in Henderson and the greater Las Vegas area, we hear about this often from patients noticing early jowling or a lower face that isn't as “snapped” as it used to be, but who aren't ready for surgery.

As a nurse practitioner who evaluates candidates for this treatment, I want to walk you through what PDO threads actually are, what the research says about how well they work, and what they can't do. This is a newer, less-studied option compared to wrinkle relaxers or microneedling, so an honest conversation matters more here, not less.

What Are PDO Threads, and How Do They Work?

PDO stands for polydioxanone — a synthetic, absorbable suture material used safely in surgery for decades. In aesthetic medicine, thin PDO threads (smooth, or with tiny barbs/“cogs”) are inserted under the skin with a fine needle, typically in the cheeks, jawline, or lower face.

The treatment works through two mechanisms:

•       Mechanical lift: barbed threads can grip surrounding tissue for an immediate, subtle repositioning — think millimeters, not inches.

•       Biostimulation: the body treats the thread as a foreign material and mounts a mild wound-healing response, including fibroblast activation and new collagen deposition. The thread fully dissolves over roughly four to six months, but the collagen response can keep developing for weeks after.

 

That second mechanism is what links PDO threads to microneedling and collagen biostimulators — all three lean on the same collagen-stimulating category of treatment.

What the Research Shows

The honest answer is: PDO threads are one of the less rigorously studied treatments in aesthetic medicine compared to injectables like wrinkle relaxers, and the evidence base, while growing, still has real limits.

•       Germani et al. 2025, Aesthetic Surgery Journal Open Forum (RCT, n=22): compared 6 vs. 12 PDO threads per side of the face, with 3D imaging at 20 and 60 days. Both groups showed an initial volume increase at 20 days, but by 60 days measurements had decreased relative to baseline — with no statistically significant difference in outcomes or satisfaction between groups. More threads didn't mean a better or longer-lasting result. Transient pain occurred in 50% of patients at 20 days, resolved by 60 days.

•       Rtail et al. 2024, The Dermatologist (complication-focused literature review): found published PDO thread complication rates exceeding 30% across the existing literature — spanning transient issues (bruising, swelling, numbness, dimpling), less common acute events (infection, nerve irritation), and rare chronic issues (contour irregularities, scarring). Most are transient, but this is a meaningfully higher published rate than typically reported for wrinkle relaxers or HA fillers.

•       Su et al. 2024, Aesthetic Surgery Journal Open Forum (preclinical, pig model): found PDO thread placement triggers measurable tissue-level changes around the thread, including gene expression shifts tied to collagen and fat metabolism — supporting the biological plausibility of the biostimulatory mechanism. This is animal, not human, data, so it speaks to mechanism, not guaranteed clinical results.

•       American Society of Plastic Surgeons, patient education materials: describes thread lift results as “subtle and natural looking,” typically lifting tissue only a few millimeters, lasting roughly one to three years — a much shorter timeline than a surgical facelift.

 

Together, this supports PDO threads as a real, biologically plausible option for mild-to-moderate laxity — but not a dramatic or long-lasting stand-in for surgery, and not one where more product reliably means more result. That's a thinner evidence picture than wrinkle relaxers, which have decades of large randomized trials behind them.

Who Tends to Be a Good Candidate

Based on the available research and clinical experience, PDO threads tend to work best for:

•       Adults roughly in their late 30s through 50s with mild to moderate skin laxity — not significant, heavy sagging

•       Patients wanting subtle refinement in the jawline, cheeks, or lower face rather than a dramatic lift

•       Patients not ready for (or not wanting) surgical facelift downtime

•       People without active skin infections, autoimmune conditions affecting wound healing, or known suture-material sensitivity

•       Realistic expectations about a temporary result (roughly one to three years), not a permanent one

 

Threads are generally not recommended for significant skin excess or severe sagging, since they reposition tissue rather than remove it — in those cases, a surgical consultation is the more honest referral.

What to Expect: Procedure and Timeline

•       Consultation and skin assessment to confirm candidacy and map thread placement

•       In-office procedure, typically 30–60 minutes, under local numbing

•       Some bruising, swelling, or a tightness/pulling sensation in the first 3–7 days

•       Initial mechanical lift is visible right away; the collagen-driven firming effect keeps building over the following 6–12 weeks as the thread dissolves

•       Results generally last 1–3 years; many patients combine or follow up with other collagen-supporting treatments to extend results

Where PDO Threads Fit Alongside Blanqa's Other Options

Because PDO threads are a newer, moderate-evidence treatment, we treat them as one option among several rather than a default recommendation. If your main concern is dynamic lines from muscle movement (forehead lines, crow's feet, the “elevens”), a wrinkle relaxer is usually the more proven starting point. If volume loss is the issue — hollow cheeks, a tired look — dermal filler has a stronger evidence base for that. If your goal is broader collagen renewal and texture over time, microneedling (alone or with a collagen biostimulator) has a deeper research base than PDO threads currently do.

For patients with true skin laxity who've already addressed lines and volume, PDO threads can be a reasonable next step, sometimes paired with microneedling or biostimulators to support the collagen response from more than one direction. The right combination depends on your skin, goals, and budget — exactly what a consultation is for.

PDO thread lifts are a legitimate, minimally invasive option grounded in real biology — but also one of the newer, less-studied treatments in our field, with a published complication rate worth taking seriously and a modest, temporary result.
