Fat Dissolving Injections: What They Can (and Can't) Do
You've done the diet. You've done the exercise. And there's still that soft pocket of fullness under your chin that shows up in every photo, no matter how much weight you lose everywhere else. If that sounds familiar, you're not imagining it — submental fat (the medical term for fat under the chin) is notoriously resistant to diet and exercise, because it responds more to genetics and aging than to calorie balance.
At Blanqa Medspa here in Henderson, fat-dissolving injections are one of the questions I get asked about most, usually with a mix of hope and skepticism. That skepticism is fair — this category of treatment has been oversold by plenty of marketing over the years. So let's walk through what the actual clinical research says these injections can do, who tends to be a good candidate, and — just as important — what they can't do.
What Are Fat-Dissolving Injections?
The active ingredient behind the FDA-approved injectable fat-reduction treatment (brand name Kybella) is deoxycholic acid — a naturally occurring molecule that your body already uses to help digest dietary fat. When injected in a concentrated, controlled dose directly into a pocket of subcutaneous fat, it breaks down the outer membrane of fat cells in that area, a process called adipocytolysis. Your body then clears the damaged cells the same way it clears any other cellular debris, over the following weeks.
Different practices may use different formulations of injectable lipolytic (fat-dissolving) treatments, so it's always worth asking your provider exactly what's in the syringe and what evidence exists for that specific formulation — that's a conversation we're always happy to have during a consultation.
What the Research Shows
This is one of the more rigorously studied injectable treatments in aesthetics — deoxycholic acid went through full FDA phase III trials before approval, which isn't true of every med spa treatment on the market. Here's what the peer-reviewed and regulatory evidence actually shows:
• Strong phase III trial results: In the pivotal REFINE-1 and REFINE-2 randomized, placebo-controlled trials (over 1,000 patients across 70 centers), 66.9–70.3% of patients receiving deoxycholic acid injections showed at least a 1-grade improvement in submental fat on a validated clinical scale, compared to just 18.7–22.4% on placebo (p<0.001 in both trials). MRI-based volume measurements confirmed the effect objectively, not just by visual grading. • High patient satisfaction: Across the phase III program, overall patient satisfaction with treatment approached 90%, and fewer than 4% of participants discontinued due to side effects — with no serious adverse events reported in the trials themselves.
• Real-world side effects are common but usually manageable: A 2024 systematic review of adverse events found that temporary bruising or swelling at the injection site is common (occurring in the large majority of patients), while more serious complications — like temporary nerve-related asymmetry in smile (about 4% of trial patients) or temporary patchy hair loss in the treated area (higher in some early male-patient series) — are uncommon and, per FDA and published follow-up data, have consistently resolved on their own, typically within weeks to a few months. • It's FDA-regulated, which matters: The FDA prescribing information spells out clear dosing limits (up to six treatment sessions, spaced at least a month apart) and specific precautions — including avoiding treatment near swallowing muscles and using caution in patients on blood thinners, since bruising occurred in a large majority of trial participants.
Are You a Good Candidate?
Fat-dissolving injections aren't the right fit for everyone, and an honest consultation should always include screening you out if you're not a good candidate — not just booking you in. Based on the clinical guidance and FDA labeling, good candidates generally:
• Have a distinct, pinchable pocket of fat under the chin or in another treatable area — not primarily loose or lax skin (injections don't tighten skin, and can make loose skin look worse if that's the real issue).
• Are otherwise healthy, with no active infection at the treatment site.
• Are not pregnant or breastfeeding.
• Don't have a history of difficulty swallowing, since the treatment area sits near muscles involved in swallowing.
• Aren't on blood thinners or have a bleeding disorder, given how common bruising is with this treatment.
• Understand this is a gradual, multi-session process — not a single-visit fix.
This is exactly the kind of thing worth discussing openly at a consultation. If fat isn't actually the cause of your fullness — if it's skin laxity instead, which becomes more common with age — a different treatment (like a collagen biostimulator or skin-tightening approach) may serve you better, and I'd rather tell you that upfront than sell you a treatment that won't deliver what you're hoping for.
What to Expect: A Realistic Timeline
• Session 1: A series of small injections into the marked treatment area, roughly 15–20 minutes. Mild swelling, bruising, and numbness in the days afterward are expected and normal — this is the most commonly reported side effect.
• Weeks 1–4: Initial swelling resolves. The treated fat cells continue to break down and clear gradually — this is not an instant result.
• Sessions 2–4 (spaced 4–8 weeks apart): Most patients in the clinical trials needed multiple sessions — commonly 2 to 4, with up to 6 allowed under FDA labeling — to reach their desired contour.
• 12+ weeks after the final session: This is when the trial data shows the clearest, most stable visible improvement — results build slowly, not overnight.
What Fat-Dissolving Injections Can't Do
In the interest of setting realistic expectations rather than overpromising: this treatment permanently reduces fat cells in a specific, targeted area — it is not a weight-loss treatment, will not meaningfully change your weight on a scale, and won't address loose or crepey skin. It also isn't a substitute for an evaluation of your overall health if fullness under the chin is a new or rapidly changing symptom, which can occasionally have causes unrelated to fat (like thyroid or lymph node changes) and deserves a clinical exam first, not just an injection.
If diet, exercise, and time haven't touched that stubborn pocket under your chin, and a consultation confirms fat — not skin or something else — is the actual cause, this is a well-studied, FDA-regulated option worth discussing.
