What Causes Spider Veins?
Spider veins (and their slightly larger cousins, reticular veins) form when the tiny one-way valves inside a vein weaken. Blood that should keep moving toward the heart instead pools and flows backward, raising pressure inside the vessel and stretching its walls until it becomes visible through the skin. A number of factors make this more likely:

How Sclerotherapy Works
Sclerotherapy is a simple, in-office procedure. A very fine needle is used to inject a sclerosing solution — most commonly polidocanol — directly into the affected vein. The solution irritates the vein's inner lining, causing it to swell and stick together (a process called endofibrosis). Over the following weeks, the closed vein is gradually reabsorbed by the body, and blood is naturally rerouted through healthy veins nearby. No incisions, lasers, or general anesthesia are involved.
What the Research Shows
Sclerotherapy is one of the more thoroughly studied cosmetic vascular treatments, with multiple randomized, blinded trials comparing sclerosing agents against each other and against placebo. Here's what four key sources found:
• The EASI Study (Phlebology): this double-blind, randomized trial followed 316 patients with spider and reticular veins across 19 German medical centers. At three months, polidocanol achieved a 96% treatment success rate with 88% of patients reporting they were “satisfied” or “very satisfied” — compared to 92% success and 64% satisfaction with sodium tetradecyl sulfate, and just 13% satisfaction with saline placebo. Results held steady at the six-month follow-up.
• Bertanha et al., JAMA Dermatology (triple-blind RCT): in 93 women treated for reticular veins, polidocanol diluted in hypertonic glucose eliminated 95.17% of treated vein length, versus 85.40% with glucose solution alone (P<.001) — with no significant difference in minor side effects like pigmentation between the two groups.
• Sclerotherapy — StatPearls (NCBI Bookshelf): this clinical reference describes sclerotherapy as the “gold standard” intervention for superficial spider, reticular, and varicose veins. Its most common side effect is temporary hyperpigmentation at the injection site, reported in roughly 30% of patients, which typically fades over the following months.
• American Academy of Dermatology: the AAD notes that sclerotherapy remains the most common treatment for visible leg veins, that spider veins typically fade within 3 to 6 weeks (larger veins take 3 to 4 months), and that most patients need 2 to 3 sessions for full results — with outcomes depending heavily on the treating provider's skill and technique.
Taken together, the evidence is consistent: sclerotherapy with modern sclerosing agents like polidocanol produces high success rates and strong patient satisfaction, and it clearly outperforms doing nothing (placebo). As with any cosmetic procedure, results vary by individual, vein size, and the number of sessions completed.
Is Sclerotherapy Right for You?
Most people bothered by visible spider or reticular veins on the legs are reasonable candidates, but a proper evaluation matters. Good candidates typically:
• Have visible spider or reticular veins without signs of significant underlying venous insufficiency
• Are bothered by the appearance, or have mild symptoms like aching or heaviness from small surface veins
• Understand that most people need 2 to 3 sessions spaced several weeks apart for full clearance
A few situations call for extra caution or a different plan altogether, and we screen for these at consultation:
• Pregnancy or breastfeeding
• A personal history of blood clots (DVT) or a known clotting disorder
• Active skin infection at or near the treatment site
• A known allergy to the sclerosing agent
• Signs of larger, underlying vein disease, which may need an ultrasound evaluation or vascular referral before cosmetic treatment
What to Expect: A Realistic Timeline
• Day of treatment: a brief in-office visit with no anesthesia required — you can walk out and resume most normal activities the same day.
• First 2–3 weeks: compression stockings and daily walking are typically recommended to support healing and reduce side effects.
